「すぐに、ただちに」という意味の副詞句。また、「同時に」という意味でも使われます。

The manager had to deal with several crises at once, leaving her no time to rest between them.
マネージャーは複数の危機に同時に対処しなければならず、その間に休む時間がまったくありませんでした。

at は古英語の æt（～のところに）に由来し、once は古英語の ānes（一度）に由来します。この二語が組み合わさり、「一度のうちに」→「すぐに」「同時に」という意味が生まれました。中英語の時代からこの形で広く使われています。
